This is a pic from my recent swimming class.

During one session, a new, submerged metal ledge was added at the start of a lane for kids. It was a thoughtful intent but rolled out with zero signage.ย
To the younger swimmers it was an "efficient" change but for another elderly swimmer with a prior knee surgery, that unannounced change was a high-impact risk. A misstep could have set her back months.
I see this "๐ ๐ฎ๐ถ๐ป๐๐๐ฟ๐ฒ๐ฎ๐บ ๐ข๐ฝ๐๐ถ๐บ๐ถ๐๐ฎ๐๐ถ๐ผ๐ป" in countless places.ย
We solve for the majority, assume readiness, and go blind to the outliers.ย

History is littered with "mostly successful" systems that failed the 1% catastrophically:

๐ ๐๐ฎ๐ฟ๐น๐ ๐๐ถ๐ฟ๐ฏ๐ฎ๐ด๐: Designed for "average" adults, they injured smaller passengers.
๐ ๐๐ ๐๐ถ๐ฟ๐ถ๐ป๐ด ๐ง๐ผ๐ผ๐น๐: Codifying historical bias, disadvantaging qualified talent.
Maturity in leadership isn't just about celebrating flow; itโs also about anticipating failure modes.
Execution without an awareness of consequence isn't scale - itโs a liability.ย
This requires slowing down enough to care before we strategize to scream "success".
Letโs stop only asking: โDid this work for most?โ
And slow down enough to also start asking:ย
โWhere are the fracture points and what is the price of the fallout?โ
